Newer Antiplatelets
نویسنده
چکیده
Many therapeutic drugs have been identified that possess clinically important antiplatelet activity. Platelet inhibition can be achieved in numerous ways (Fig. 1), including inhibition of platelet cyclooxygenase, inhibition of ADP receptors or inhibition of the platelet glycoprotein (GP) IIb/ IIIa receptor. Pharmacological inhibition of platelet function can also be achieved by interference with the function of the platelet glycoprotein Ib-IX receptor, synthetic peptides to the A1 Von Willebrand’s factor domain, recombinant Von Willebrand’s fragments covering the A1 domain, inhibition of the thromboxane synthase, blockade of endoperoxidethromboxane receptors, modulation of platelet adenyl or guanyl cyclase and peptides that bind to but do not activate the platelet-receptor domain that interacts with thrombin. This section focuses on new antiplatelet agents or new data with existing drugs which has clinical implications.

Prevention of Stroke by Antithrombotic Therapy in Patients with Atrial Fibrillation.
Atrial Fibrillation (AF) is the most common clinically significant sustained cardiac arrhythmia, a major risk factor for strokes whether it is symptomatic or silent. The older CHADS2 score and the newer CHADS2-VASc are well validated to determine stroke risk and guide initiation of antithrombotic therapy, but haemorrhagic risk has to be respected as well, and scores such as HAS-BLED should be w...
متن کاملCurrent oral antiplatelets: focus update on prasugrel.
Platelet activation and aggregation plays an integral role in the pathogenesis of acute coronary syndrome (ACS). The mainstay of ACS treatment revolves around platelet inhibition. It is known that greater platelet inhibition results in better ischemic outcomes; hence, focus in drug development has been to create more potent inhibitors of platelet aggregation. Prasugrel, a potent, third-generati...
متن کاملAppropriate Anti-Thrombotic/Anti-Thrombin Therapy for Thrombotic Lesions
Managing coronary thrombus is a challenging task and requires adequate knowledge of the various antithrombotic agents available. In this article, we will briefly analyze the risk-benefit profile of antithrombotic agents, with critical analysis of the scientific evidence available to support their use. Since thrombus consists of platelets and coagulation cofactors, an effective antithrombotic st...

OBJECTIVE To compare the effects of antiplatelets and anticoagulants on stroke and death in patients with acute cervical artery dissection. DESIGN Systematic review with Bayesian meta-analysis. DATA SOURCES The reviewers searched MEDLINE and EMBASE from inception to November 2012, checked reference lists, and contacted authors. STUDY SELECTION Studies were eligible if they were randomised...
متن کاملEffect of antiplatelet therapy for endovascular coiling in aneurysmal subarachnoid hemorrhage.
BACKGROUND AND PURPOSE Antiplatelets are frequently used during or after endovascular coiling of aneurysm in patients with subarachnoid hemorrhage (SAH). This strategy is based on uncontrolled case series including also patients with unruptured aneurysms or other lesions. We collected data on effectiveness of antiplatelets in patients with SAH. METHODS All 43 participating centers in the Inte...
متن کاملAntiplatelet and Statin Use in US Patients With Coronary Artery Disease Categorized by Race/Ethnicity and Gender, 2003 to 2012.
Antiplatelets and statins are efficacious for preventing future cardiovascular events in patients with coronary heart disease. Disparity in cardiovascular outcomes exists by race/ethnicity and gender; however, few studies have explored potential disparities in long-term antiplatelet and statin use by race/ethnicity and gender. We conducted a repeated cross-sectional analysis using the nationall...
